Please understand that plans vary within each insurance carrier. Please contact your insurance provider to determine if our services are covered and fill out the insurance verification form provided. Your insurance provider will be able to inform you if Naturopathic physicians (and specific Naturopathic physician) are covered under your plan. Please clarify any out-of-pocket expenses (ie. deductible) and co-pays prior to your visit. We are not responsible for charges incurred if you do not check your coverage.

This does not guarantee that your insurance will cover the entire cost. We are contracted with the following, but not limited to:
Our office will submit claims to the insurance company. At your office visit, you will be required to pay any co-pay requirements for the appointment that day. For return patients, if there are previous services you have received that are not covered by your insurance, you will be asked to pay this balance prior to your next appointment.
If you have insurance that we are not contracted with, you can contact your plan to see if you have out-of-network benefits by calling the number on the back of your card. It is possible that your insurance will still cover some of the cost of your visit.
Primary Care Out-of-Pocket Fees:
Pricing for visits varies depending on complexity of the complaint(s) and time spent with the provider.
New Patient Visit (60-75min): $400
Return Patient Visit (30-45min): $280
Phone Consult *For established patients only: $50+ for 0-30 minutes
Telemedicine: $90/30-45 min
E-mail correspondence: $15 per email exchange
House Calls *For established patients only: $250/45 min appt. (may be able to bill to insurance, depending on plan)
*You receive a $20 discount for provider services if you pay at the time of service regardless of payment method.

Non-covered Services Fees:
Some of our services are not covered by insurance. *Primary care discount does not apply to additional services fees whether purchased in conjunction or at a later time. 3% merchant fee will apply to all card transactions.
We value your time and understand that change happens. We ask that you give us 24 hours notice if you wish to cancel or reschedule your appointment. The fee is $65 for changes to your appointment less than 24 hours of the scheduled time or failing to show.
